Moving On Tyne & Wear (MOTW) is joint-funded by The National Lottery Community Fund and European Social Fund, and aims to support people with a range of complex needs across Tyne and Wear to move towards or into training and employment.

This project is led by Mental Health Concern (MHC) and supported by 8 partner organisations, including Groundwork NE & Cumbria.

About the role

We are looking for a creative, enthusiastic individual to join our Moving On Tyne & Wear team as a Health Pathways Officer.

The role will support participants with Autism or LLDD to address health barriers and move them towards training and employment. The Health Pathways officer will offer one-to-one support to help participants overcome their health barriers, working in partnership with other providers to tailor packages of support suited to individual’s needs allowing them to better manage their conditions and move towards, or into, employment.  The ability to innovate and inspire participants to achieve positive outcomes and act as an advocate of the services presenting a professional image of Moving on Tyne & Wear at all times is essential.

Appointment to this post is subject to an enhanced disclosure endorsed by the Disclosure and Barring Service.

About you

We are seeking someone with experience of working with vulnerable people in job-seeking and/or other community support settings. You will have worked in partnership to deliver project outputs and outcomes. You must have the experience and ability to innovate and inspire participants to achieve positive outcomes. Qualifications in information, advice and guidance, employment-related services, or similar, at Level 3 or above are essential.
